Association of vitamin D deficiency with the risk of metabolic syndrome in the elderly

Abstract. The aim of the study was to assess the relationship between vitamin D deficiency and the risk of metabolic syndrome (MS) in elderly patients.
Material and methods. 64 patients with age from 60 to 70 years were included in the trial. General obesity was assessed by body mass index (BMI), abdominal obesity – by waist circumference (WS). Blood serum levels of glucose, insulin, 25(OH)D, total cholesterol, high-density lipoproteins (HDL), low-density lipoproteins (LDL), and triglycerides (TG) were determined. Level 25(OH)D was classified as adequate (≥30 ng/ml) and insufficient (<30 ng/ml).
Results. 62,5% (n=40) of the subjects included in the survey had metabolic syndrome. Patients with an insufficient level of vitamin D had a metabolic syndrome 39,7% more often than patients with an adequate level of it. Patients with insufficient vitamin D level had higher BMI values (Δ14,5%; p <0,05), WS (Δ8,7%; p <0,05). When assessing insulin resistance (HOMA-IR), an increase in this index (Δ11,7%; p <0,05) was noted in patients with a lack of vitamin D. When assessing fat metabolism, vitamin D levels were inversely related to total cholesterol (Δ8,4%; p <0,05), TG levels (Δ14%; p <0,05), and a direct relationship was observed with HDL (Δ8,9%; p <0,05). The level of systolic blood pressure was significantly higher (Δ8,3%; p <0,05) in patients with a lack of vitamin D. The number of MS components was also 25,5% (p <0,01) higher in patients with insufficient vitamin D (2,31±1,30) compared to patients with adequate vitamin D (1,72±1,16). A significant positive correlation of the level of VD with HDL (r=0,65; p <0,05) and negative with WS (r =-0,71; p <0m05), HOMA-IR index (r=- 0,77; p <0,05), TG content (r=-0,63; p <0,05) in serum was established.
Conclusions. Thus, the results of our study indicate an increased risk of the developing of MS in elderly patients with vitamin D levels <30 ng/ml. We have demonstrated that the concentration of 25(OH)D serum is inversely associated with insulin resistance and MS.
