Features of endothelial dysfunction and lipid metabolism on myocardial remodeling in patients with arterial hypertension in the highlands

Objective: to study the concentration of nitric oxide metabolites and lipid profile features in left ventricular remodeling and the relationship between them in patients with arterial hypertension, residents of the highlands.
Material and methods. The study included 200 indigenous people in the Elbrus highlands: 130 patients with grade 1 and 2 hypertension and 75 healthy individuals. All examined patients underwent clinical, instrumental and biochemical examination, including: echocardiography and evaluation, determination of plasma lipids and stable metabolites of no in the blood.
Results. Myocardial remodeling was revealed in 46% of hypertension patients. With an increase in the degree of hypertension, a progressive decrease in NO production, an increase in lipid metabolism disorders, changes in myocardial geometry, an increase in the prevalence of concentric (KG) and eccentric (EG) left ventricular (LV) hypertrophy and a decrease in normal LV geometry (NG) were revealed.
Conclusion. The obtained data indicate a greater change in myocardial geometry, more frequent development of CD, KG and EG and hemodynamic disorders in patients with reduced no production in combination with a disturbed lipid profile and their close relationship with each other.
