Community-acquired pneumonias in rural hospitals of Stavropol region: from antibiotic therapy to the structure of bacterial causative agents

The aim is to study the peculiarities of pharmacotherapy for patients with community-acquired pneumonia (CAP) and the structure of bacterial causative agents of CAP in day-and-night duty hospitals in one of the rural areas of the Stavropol territory.
Material and methods. Medical histories of 1134 patients with confirmed diagnosis of CAP who were treated in one main district and three local district hospitals of the Shpakovsky territory in 2010–2018 were studied. All patients included in the study underwent standard clinical laboratory and instrumental examinatiins. The treatment was carried out in accordance with current recommendations.
Results and conclusion. The incidence of CAP in the district was 0,8% of the total population of the region (n=141210) and 40% of the total number of registered cases of respiratory organs pathology (n=2833) for 2010–2018, which indicates the urgency of the problem of CAP for rural population. In the sputum of the studied patients, both pathogenic and conditionally pathogenic microorganisms were found. The main representatives of the flora during sputum sowing were Str. pneumoniae, Enterobact. spp., Str. spp., S. aureus and H. influenza. Microorganisms were found both as monocultures (81%) and also as a part of associations (19%). In hospitalized patients, depending on the severity of the CAP course during starting monotherapy, third-generation cephalosporins were prevailing. In combined pharmacotherapy, the drugs of choice were various combinations of ß-lactam ABPs, as well as a combination of cephalosporins with β-lactams, which were used at medium doses parenterally according to the generally accepted scheme, which corresponded to the standard of primary health care for pneumonia and provided a recovery in 86,2% cases.
