Is there an alternative to multi-target fixed combinations for primary prevention of cardiovascular disease?

Abstract. The article presented to the reader is devoted to the problem of combination therapy of arterial hypertension. The article updates the historical retrospective of the use of combined drugs to lower blood pressure in our country. The variety and number of such agents previously used to correct arterial hypertension are highlighted in detail. Many of the drugs that were widely used in clinical practice in the past are currently only of historical importance, since they do not have a satisfactory efficacy and tolerance profile, which has been confirmed by numerous scientific studies. The stages of the introduction of «new» antihypertensive drugs during the 70–90s of the last century are consistently considered. The approaches of the domestic health care system and our foreign colleagues in the treatment of arterial hypertension are compared. The paper discusses combined low-dose antihypertensive therapy as an important option for the treatment of isolated and complicated hypertension. Special attention in the article is paid to multitarget therapy, as the most important successor to the «polypill» concept in the context of primary cardiovascular prevention.
