Central arterial pressure measurement as a promising method for identifying the initial stage of arterial hypertension

Abstract. Aim: to clarify the mechanisms of formation of systolic-diastolic arterial hypertension (SDAH) by measuring central arterial pressure.
Material and methods. We examined 332 people aged 30–60 years: 115 patients with normal blood pressure, 105 with high normal blood pressure and 112 with the first degree of SDAH. All patients underwent applanation tonometry with SphygmoCor device (AtCorMedical, Australia) with the determination of the levels of central blood pressure and indicators reflecting the state of the central hemodynamics.
Results. The measurement of central blood pressure (in the aorta) makes it possible to determine the initial signs of arterial hypertension. An increase in blood pressure to the high normal stage occurs both due to pulse and diastolic blood pressure, and conversion to the SDAH happens due to an increase in diastolic blood pressure in young and middle-aged patients. Augmentation of the diastolic part of the pulse wave along its entire length plays an important role in these processes. An increase in diastolic blood pressure in the early diastole improves heart and other central organs blood supply.
Conclusion. Augmentation of the systolic part of the central pulse wave leads to an isolated increase in central pulse and systolic blood pressure. In the presence of several risk factors for an increase in central pulse wave augmentation (increased arterial stiffness, low height, female sex), an isolated, hidden, central high-risk hypertension can be formed.
