Predictive self-descriptiveness of cognitive status assessment in polymorbid patients with chronic heart failure

Abstract. The purpose of the study: evaluation of the prognostic potential of the Mini Mental State Examination (MMSE) test in relation to a fatal outcome in patients with chronic heart failure (CHF).
Material and methods. The MMSE questionnaire was used to study the prevalence and severity of cognitive dysfunction in patients with CHF. We studied the predictive potential of cognitive dysfunction in relation to the death of patients with CHF within 1 year from the date of inclusion in the study.
Results. An increase in the severity of cognitive deficit with an increase in CHF stage, a negative correlation of MMSE indices with the concentration of the N-terminal fragment of brain natriuretic peptide (NTproBNP), pulmonary artery pressure and left atrial index were revealed. In addition, an increase in the rate of death of patients with a decrease in MMSE to the level of dementia was demonstrated. It was found that the level of NTproBNP and the rate of death in patients who did not perform MMSE were significantly higher than those in patients who performed this test.
Conclusion. The established facts allow us to consider a decrease in the MMSE index in patients with CHF as an indirect sign of its progression. The fact of non-fulfillment of MMSE can be considered as an indirect reflection of the severe course of CHF and a predictor of its unfavorable outcome.
