Prediction of the complicated course of acute myocardial infarction in patients with chronic obstructive pulmonary disease

Abstract. The aim of the study was to predict the development of a complicated course of myocardial infarction (MI) (the likelihood of developing complications – pulmonary edema and cardiogenic shock) in patients with concomitant chronic obstructive pulmonary disease (COPD).
Material and methods. The study involved 60 male patients hospitalized with a diagnosis of acute MI with COPD. Determination of the Klotho protein level (ng/ml) in plasma samples was carried out by enzyme-linked immunosorbent assay using a commercial test system «Klotho (KL)». For each predictor of the multiple logistic regression model were calculated: β-coefficient; standard error; Wald test statistics; p – values of probability (statistical significance of the relationship of predictors with the response variable); odds ratio and 95% confidence interval (CI).
Results. The keratinous level of the Clotho protein was established, which was significant for the prognosis of the development of complications (cardiogenic shock, pulmonary edema) of the acute period of MI with COPD, which was 0,185 ng/ml.
Conclusion. The developed algorithm for predicting the development of MI complications in patients with COPD has a high sensitivity and prognostic significance. Information about the expected high risk of complications (cardiogenic shock, pulmonary edema) will help to purposefully select the amount of preventive and therapeutic measures in patients with acute MI with COPD to minimize this risk.
