Integral hematological indices of intoxication as markers of complicated course of myocardial infarction in patients with chronic obstructive pulmonary disease

Abstract. Aim: compare the integral hematological indices of intoxication (leukocyte index of intoxication and white blood cell shift index) in patients with myocardial infarction (MI) against the background of chronic obstructive pulmonary disease (COPD), depending on the presence of complications in the early period of infarction.
Material and methods. 225 MI patients were examined, 195 of them developed COPD. Among patients with MI, 95 (73%) had no complications in the early period of the disease, and 35 (27%) had complications in the early period. In the group of MI patients with COPD, the number of patients with uncomplicated and complicated infarction was 114 (58,5%) and 94 (41,5%), respectively. The 2 comparison groups included 110 somatically healthy individuals and 104 non-acute COPD patients. The absolute content of white blood cells (CL/l) was determined in a two-chamber Goryaev grid. The ABX PENTRA 120 hematological analyzer was used for cell registration. The leukocyte intoxication index, lymphocyte index and white blood cell shift index were calculated using special formulas.
Results. Based on the results of our study, we can conclude that the dependence of the values of hematological indices on the presence of complications in the early period was detected only in patients with MI. This applied to all the indexes analyzed. The maximum values of leukocyte intoxication index, lymphocyte index and white blood cell shift index were obtained in the group with a complicated course of MI. In patients with MI with COPD, none of the analyzed hematological indices showed dependence on the presence of complications in the early period.
Conclusion The absence of dependence of cellular reactivity indices on the presence of early complications in patients with MI against the background of COPD indicates a change in the reactivity of the body in the presence of concomitant COPD in a patient with MI and requires the search for sensitive markers of endogenous intoxication syndrome in comorbid patients.
