Peculiarities of functional condition parameters in patients with myocardial infarction during annual observation depending on type 2 diabetes mellitus presence

Abstract. The aim was to found the peculiarities of the functional condition (FC) in myocardial infarction (MI) patients in case of presence/absence of type 2 diabetes mellitus (DM 2) at different periods of annual observation.
Material and methods. The study included 111 persons with MI (age 40–65 years), divided into groups: I – MI without DM (n=81), II – MI and DM 2 (n=30). FC was assessed on the 3rd, 8th and 14th days, 6 and 12 months after MI according to the following characteristics: A – adaptation of the cardiovascular system, B – autonomic regulation, C – central regulation, D – psychoemotional state, H – an integral indicator of health.
Results. A decrease in the value of the PC parameters relatively to the norm was fixed at all stages of the annual observation. In a comparative analysis of the results of patients in groups I and II, it was found that in group I the value of A, B, C, D, H is 3–12% higher than in group II. Dynamic changes in patients of groups I and II from hospital to post-hospital stages were expressed in an increase in C, D, H values and a decrease in B. During the first months, in MI patients the influence of the sympathetic part of the autonomic nervous system sharply prevails, further on the background of a favorable course rehabilitation and selection of the correct medicamentous therapy, the vegetative balance is leveled and/or sympathicotonia becomes less expressed. During the observation, the course of rehabilitation was favorable in all examined patients, however, in patients of group I, significant dynamic changes of FC parameters were developing faster than in group II.
Conclusion. The presence of DM 2 in patients with MI slows down FC restoration at hospital and outpatient stages of rehabilitation.
