Длительное системное применение глюкокортикоидов: на что обратить внимание врачу общей практики?

Аннотация. Глюкокортикоиды (ГК) являются важными лекарственными средствами в лечении воспалительных ревматических заболеваний опорно-двигательного аппарата благодаря их быстрому и мощному противовоспалительному действию. В то же время длительное применение ГК сопряжено с хорошо известными рисками, что привело к международному консенсусу о том, что их следует назначать в минимальной эффективной дозе и в течение кратчайшего периода времени, а затем прекращать применение, когда это клинически возможно. Современные рекомендации предписывают кратковременное использование ГК в минимальной эффективной дозе, часто возможной за счет сопутствующего применения противоревматических препаратов, модифицирующих течение заболевания, или иммуносупрессивных средств. Однако данные реальной клинической практики показывают, что хроническая терапия ГК остается распространенным явлением. В этом обзоре обобщены текущие рекомендации, подчеркивается разрыв между руководствами и клинической практикой, а также обсуждаются нежелательные эффекты ГК, необходимость мониторинга их приема, возможности измерения кумулятивной токсичности, роль врача общей практики в преодолении сложностей при использовании этого класса препаратов.
