Современные возможности оценки и коррекции структурно-функционального состояния диафрагмы у больных с хронической сердечной недостаточностью

Аннотация. Одышка и снижение переносимости физической нагрузки – частые проявления хронической сердечной недостаточности (ХСН), в основе которых может лежать дисфункция диафрагмы. Цель обзора – обобщить данные о структурно-функциональном состоянии диафрагмы при ХСН, ее роли в развитии симптомов заболевания, методах диагностики и коррекции. Поиск релевантных источников проводился в PubMed/MEDLINE и eLIBRARY.RU; из 263 найденных публикаций в качественный анализ были включены 39. Большинство исследований показало связь дисфункции диафрагмы с выраженностью одышки и снижением переносимости нагрузки. Рассмотрены патофизиологические механизмы дисфункция диафрагмы, диагностические возможности ее ультразвукового исследования, определения максимального инспираторного давления, спирометрии и кардиопульмонального нагрузочного тестирования, а также их прогностическое значение. Оценка функции диафрагмы может дополнять стандартное обследование пациентов с ХСН.
