Informative value of the domestic questionnaire for assessing adherence to treatment and its correlation with the criteria of life quality and mental state in patients who underwent various revascularizing coronary interventions

Abstract. In the current conditions of fighting with mortality from cardiovascular diseases, the role of preventive and rehabilitation measures is increasing.
The aim: to evaluate informative value of Russian universal questionnaire for Quantitative Assessment of Adherence to treatment (QAA-25) in patients who underwent coronary revascularization and to compare its results with the criteria of life quality and severity of anxiety and depressive disorders (ADD) when developing an individual program of cardiac rehabilitation at the outpatient stage.
Material and methods. 85 patients were examined one month after primary coronary revascularization; they were referred to the outpatient stage of cardiac rehabilitation. The age of the patients ranged from 42 to 79 years (mean age 60.8 ± 7.2 years). The participants were divided into 2 groups: 1st – 35 (41.2%) persons after percutaneous coronary interventions with stenting; 2nd – 50 (58.8%) persons after coronary artery bypass grafting. Patients were surveyed using QAA-25 questionnaire, Hospital Anxiety and Depression Scale (HADS), and quality of life was assessed using SF-36 scale.
Results. Correlations were found between older age and low levels of adherence to medical care (p < 0.001), lifestyle modification (p = 0.007), drug therapy (p < 0.001), and integral adherence to treatment (p < 0.001). The frequency and severity of depression were also associated with low adherence to treatment. An inverse correlation was found between the indexes of mental health, physical component of health, psychological component of health by SF-36 scale and the frequency of ADD according to HADS scale.
Conclusion. QAA-25 questionnaire in patients who underwent coronary revascularization showed a high degree of validation and the presence of correlations with ADD. An integrated assessment of treatment adherence, mental state and quality of life can be used to develop an individual cardiac rehabilitation program for such kind of patients.
