Syndrome of emotional burnout and stress levels in the workforce in Russia, according to a population-based prospective study

Objective: to study of detection frequency of burnout syndrome and stress on representative samples in production teams in different regions of Russia.
Material and methods. In the years 2010 and 2011 in five regions of Russia was carried out screening on the full (representative) sample of the organized populations (at least 500 people in each, the response to the survey, 80 to 90%). In all cities we have carried out epidemiological standardization of all researchers who conducted this screening: standardization of blood pressure measurement, anthropometric data, correct filling of all questionnaires. The organized populations taken for research were workers of large industrial enterprises in 4 cities. The exception was the population in Nalchik, where the University teachers (410 people) and public transport drivers (80 people) were examined. A total of 2227 people were examined: 1336 women and 891 men aged 25–75 years. In 2012–2014 we conducted a prospective (in 3 years) population-based study in the cities of Nizhny Novgorod, Nalchik, Veliky Novgorod and Omsk. Screening of the same organized population spent at 1202 persons (response 64,7%). Methods for health assessment were assessment by questionnaires to identify socio-economic status, somatic and psychological state of health, measured blood pressure, heart rate, height, body mass index, waist and hip circumference. Mainly questionnaires were designed for self-completion, sometimes the help of the interviewer (Nalchik) was used. These methods were used for population scientific research for the first time. Mathematical and statistical processing of the data was carried out using SAS statistical packages.
Results. All surveyed were interviewed about the level of education, well-being. Persons with higher education ranged from 29,3 to 82%. Self-assessment of the level of well-being was low in a very large part of the surveyed: from 10,4 to 40,4%. Quite often there was exposure to stress. The so-called «possible stress» on a scale perceived stress-10 (PSS-10) was determined in young men – 2,8 and 8,4% of women. But with age the spread of this condition increased dramatically – to >13 and >46% in men, as well as to >20 and >55% in women of mature and elderly age. Finally, «defenite stress», i.e. a condition requiring the intervention of a psychologist, and in some cases a psychiatrist, was observed in ≈5% of adults and 11% of men and 21% of elderly women. Low values of [0–1] the burnout syndrome (typical for the emotionally healthy part of the population) were observed in 47,6 and 58,7% of the youngest (25–34 years) men and women. With age, this percentage decreases to 23,3 and 32,4% in 35–44 years and to 9,1 and 10,9% in 65–74 years. But the frequency of less favorable values of the burnout syndrome increases [2–6 points], this applies to persons of mature and especially elderly age. In 35–44 years, such values were observed in more than 60% of cases, and in the elderly in more than 80%. The obvious burnout syndrome [7–10 points] diagnosed of course significantly less, but still have ≈15% of men and 6% to 8% of women middle-aged and elderly (65–74 years).
Conclusion. Thus, it can be seen that in the population of workers of any specialty there is a high susceptibility to stress, they have socio-economic, psycho-social and moral problems, which led to the development of emotional burnout syndrome of varying severity.
