Abstract. The article reviews the issues of pathogenesis, clinic, diagnosis and treatment of chronic brain ischemia. Highlights of the development of «small vessel disease», endothelial dysfunction, processes of lipid peroxidation, apoptosis as well as neuroplasticity. The necessity of joint management of patients with chronic cerebral ischemia by neurologists and doctors of other specialties and the appointment of individualized pathogenetic therapy are noted.
