Abstract. The article describes a clinical case of a 67-year-old male patient with a rupture of the interventricular septum and the anterior wall of the left ventricle in the area of acute vast transmural myocardial infarction. This condition took place against the background of long-term arterial hypertension. The disease was complicated by cardiac tamponade, which was the immediate cause of death. The paper also provides literature data on the incidence of this condition, interventricular septal rupture topography, and the frequency of intravital diagnosis of septal defects using echocardiography. The interventricular septal rupture occurred in the middle third of the anterior part of the septum. The lack of dynamic follow-up of the patient on an outpatient basis, in this case, played an unfavorable role that resulted in the patient’s death.
