Sartans: from treatment of hypertension to management of cardiovascular risk

Angiotensin-receptor blockers (ARB) are one of four drug classes recommended for the initial treatment of hypertension. These medications are commonly used not only for hypertension, but also for heart failure and chronic kidney disease. Fimasartan, the ninth and latest ARB, is a pyrimidin-4(3H)-one derivative of losartan, which has replaced the imidazole ring. Fimasartan is a selective angiotensin II type 1 receptor antagonist. It provides a noncompetitive, insurmountable binding with this receptor. Preclinical studies have shown some potential pleiotropic mechanisms, including anti-inflammatory and organ-protection effects. In phase II clinical studies, fimasartan showed an excellent safety profile and at a dosage range 60–120 mg once daily showed an antihypertensive effect over 24 h. In phase III confirmatory clinical studies conducted using other Angiotensin Receptor Blockers as comparators, fimasartan was non-inferior to losartan and candesartan in reducing blood pressure and proved an effective blood pressure lowering effect greater than that of valsartan.
