Abstract. Psoriatic arthritis (PsA) attracts the attention of researchers and practitioners for many reasons, including such associated problems as high comorbidity and cases of failure to achieve treatment goals despite adequate therapy. Current article presents a clinical example of dufficult-to-treat PsA in a young patient having signs of metabolic syndrome (MS). Currently, he is becoming therapy with the sixth genetically engineered biological drug (the first was prescribed in 2013), and for a long period of time he cannot achieve satisfactory control over his body weight. It could be assumed that obesity and MS make a significant contribution to the impossibility of achieving stable remission of PsA. Probably, the implementation of dufficult-to-treat PsA criteria into clinical practice in combination with the use of an interdisciplinary approach to the management of such kind of patients will improve the efficacy of treatment regimens, prognosis and life quality of patients with that disease.
